Use for Leak Repairs made by a licensed plumber or contractor or new sod:

For leak repairs made by homeowner or someone other than a licensed plumber or contractor, a notarized affidavit of the repair stating when and where the leak was repaired is required:
Also, with all adjustment applications,dated and paid receipts for parts is required. These may be emailed to HCUDCS@co.hernando.fl.us or be mailed to or brought to our office at 15365 Cortez Blvd. Brooksville, FL 34613.
Please note, it is the customers responsibility to turn off autopay during the adjustment review process to avoid the total account balance be auto paid.
