Driver License

Applicants who are applying for an original or renewal Florida Driver License must provide:

Sample Drivers License

  • United States Birth Certificate, valid United States Passport, or Proof of Legal Presence
    *A birth certificate must be issued by a government agency. Hospital birth certificates cannot be accepted.
    • For U.S. citizens, if the documentation submitted reflects any name change since birth, an original or certified copy of your Marriage License(s), Divorce Decree or Court Order, must be submitted. In some instances, more than one document may be required to show the chain of name changes. Photocopies cannot be accepted.
      • Non-U.S. citizens will be issued with the name on Legal Presence documents. 
    • Documents MUST show the reflection of the name change.
  • Proof of Social Security Number (if issued, Chapter 322, Florida Statutes, requires the Department to see proof of social security number for the issuance of Driver License or Identification Card)
    • The name assigned to the social security number must match the name that will appear on the Florida Driver License or Identification Card.  If you have recently changed your name, you must first update your records with the Social Security Administration. Wait 24 hours after you update your records before you apply for your Driver License or Identification Card.
  • Two proofs of residential address

All applicants applying for an original Florida Driver License:

  • Must show proof that they have completed a Traffic Law and Substance Abuse Education Course
  • Be required to pass vision and hearing tests
  • Road rules and road sign tests
  • A driving test.

Learner's Permit

Applicants applying for a Learnerโ€™s License must be at least 15 years old and must present a parental consent form (go to the complete list of Learnerโ€™s License requirements). The same identification requirements for citizens and Proof of Legal Presence for non-citizens are required to obtain a Learner's License.

Originals with a valid out-of-state Driver License and ID card (United States Citizens)

Before a Driver License or Identification card can be issued, Florida law requires all customers have:

  • United States Birth Certificate, valid United States Passport, or Proof of Legal Presence

Florida Identification Card

For an original or renewal Identification Card, apply in person at a Driver License office. No test is required. Before an Identification Card can be issued, Florida law requires from all customers:

Any person who is 5 years of age or older, or any person who has a disability, regardless of age, who applies for a disabled parking permit under Section 320.0848, may be issued an Identification Card.

Renew or Replace an Identification Card

You may apply online using the MyDMVPortal website for the following Identification Card services:

  • Renewal ID Card
  • Replacement ID Card if yours was lost/stolen/destroyed or if you need an address change

Note
: Giving false information on an application for a driver license or identification card is a third degree felony which may result in arrest, and includes a one-year suspension of the driving privilege. Proof of correct address may be required, for example, cancelled mail envelope received.
